• Succession certificate for orphan children and two passport-size photos.


• Copies of CNIC/B-Form for all children of the martyr and the death certificate.


• Copy of CNIC of the martyrs widow mother or guardian.


• Employment certificate from the relevant department.


• Academic records of the martyrs orphaned children with contact information for their educational institutions.


• Verification details from the current school, college, or university for school-going children.


• Copy of the martyrs service card, payslip, or service book.


• Copy of the CNIC and contact details of the family focal person, specifying their relationship with the martyr.
